The present case report aims to describe the occurrence of a multiple pregnancy in a cow subjected to hormonal treatment for fixed-time artificial insemination (FTAI). A crossbred beef cow (Angus × Nelore), 6 years old, with a history of four previous calvings, was managed during the 2022/2023 breeding season. The hormonal protocol consisted of the insertion of an intravaginal device impregnated with 1 g of progesterone and intramuscular (IM) administration of 2 mg of estradiol benzoate. After eight days, the device was removed and the cow received 500 µg of sodium cloprostenol associated with 1 mg of estradiol cypionate IM. Approximately 48 to 52 hours later, the cow was inseminated with semen from an Angus bull, followed by IM administration of 25 µg of lecirelin acetate. Pregnancy was diagnosed by transrectal ultrasonography 30 days after FTAI. Gestation lasted 301 days, and parturition took approximately 5 hours; the first two calves were delivered naturally, whereas the last two required obstetrical assistance with manual traction.
